ASCO has formally recommended that the Centers for Medicare and Medicaid Services use the Quality Oncology Practice Initiative® (QOPI) as a model clinical registry through which practices can meet their Physician Quality Reporting System (PQRS) requirements. The society made this recommendation in submitted comments in response to the agency’s request for information on how practices can meet their PQRS requirements by participating in clinical registries.

The first three installments of ASCO’s new payment reform series provide context and background on the growing call for reform of the Medicare physician payment system. Rising healthcare costs, along with misaligned payment formulas, have triggered an unprecedented national dialogue on the need to change the way Medicare reimburses physicians for patient care. Congress, the Administration and the oncology community are seriously examining options for restructuring care delivery and reimbursement to ensure higher-quality and more cost-efficient patient-centered care.

An analysis of self-reported data from outpatient oncology practices that participated in the American Society of Clinical Oncology’s Quality Oncology Practice Initiative (QOPI®) found a significant increase in performance on quality measures for oncology care over a four-year period, especially in adopting new clinical practices. The study was published on March 11, in the Journal of Clinical Oncology.

A provision in the American Taxpayer Relief Act of 2012, which passed in the waning hours of the past year to avert the “fiscal cliff,” may allow oncology practices to meet their Physician Quality Reporting System requirements by participating in ASCO’s Quality Oncology Practice Initiative® (QOPI) starting in 2014.

ASCO participated in a Senate Finance Committee roundtable on reforming Medicare’s payment fee-for-service model on July 11. In testimony before the committee, ASCO representative Barbara McAneny, MD, stressed the importance of integrating a strong cancer-focused quality system into all payment models, allowing physicians to lead any move to a new model of care delivery and payment, and of testing and transitioning any new payment system to fully understand any implications on practice and patients.
